單位登記憑證復(fù)印件。企業(yè)應(yīng)出具公司《營(yíng)業(yè)執(zhí)照副本》，其他單位應(yīng)出具《事業(yè)單位法人證書》等憑證材料。如任職單位是外商投資企業(yè)的，企業(yè)還應(yīng)出具《外商投資企業(yè)批準(zhǔn)證書》或《外商投資企業(yè)設(shè)立備案回執(zhí)》、《外商投資企業(yè)變更備案回執(zhí)》。

申請(qǐng)人個(gè)人簡(jiǎn)歷（18周歲至今，要求年份和月份之間連續(xù)）。
